多切層電腦斷層於頭頸部腫瘤鑑別診斷的檢查策略

Strategies of Examination Techniques for Differential Diagnosis of Head and Neck Tumors by MDCT

摘要


隨著電腦斷層技術的進步,影像的品質日益受重視,本研究結合了TOSHIBA Aquilion one 320切MDCT以及針對頭頸部腫瘤發展出一套技術來提升腫瘤在影像上的正確診療率,利用雙頰充氣技術(The Puffed Cheek Technique)可清楚在影像上分辨出口腔內複雜解剖構造及各個黏膜,瓦沙爾氏法(Valsalva's Maneuver)可針對下咽部的腫瘤做出更清楚的腫瘤定位,E"-發聲動作("E"- Phonation)可清楚分辨真聲帶及假聲帶位置,對於聲門及聲帶的腫瘤可更容易的診斷出病兆,研究結果顯示在取樣總病人數180位患有頭頸部癌的病患,發現有顯著的提高影像的診斷價值,證實此方法在臨床上的可行性。

With the technical advancement of computed tomography, the quality of imaging has been a growing concern. Our study was to determine the effect of using the puffed cheek technique and Valsalva's maneuver on diagnostic accuracy of head and neck malignancy on Toshiba Aquilion one 320 MDCT, focusing on anatomic structures in the oral cavity and the hypopharynx. For the 180 patients enrolled in our study, we concluded that the maneuvers were feasible and greatly improved the diagnostic accuracy of head and neck malignancy on CT.
