Research on the Difference between Promoting the Development of Patient Safety Culture and Creating a Reliable Medical Professional Environment

促進病人安全文化發展與營造可靠的醫療職業環境之差異性探討

Objective: Nowadays, the primary issue facing medical institutions is the long-term overwork of medical staff. This study wants to know whether the promotion of patient safety culture measured by "New Safety Attitudes Questionnaire" is appropriate. If relevant information can be collected quickly after patients' accidents, we can take the lead to deal with the risks of disease and safety and reduce the probability of accidents. Experimental methods: This study used the hospital patient New Safety Attitudes Questionnaire (New SAQ) developed by Dr. Sexton of Duke university as the model to explore the patient safety culture. The medical staff of a medical center in Taipei, Taiwan in 2017 to 2019 were selected. A total of 1284 questionnaires were sent out, 1020 of which were effectively recovered, and the effective recovery rate was 79.44%. To explore the degree of correlation between various aspects of patient safety culture and analyze the degree of influence of various aspects of patient safety culture on overall medical staff satisfaction. Results: This study shows that there is a significant negative correlation between stress perception and work status, indicating that the higher the pressure is, the worse the work status is. In addition, the performance of work conditions in various aspects of the patient safety culture was not satisfactory, indicating that the majority of health care workers believed that the current working environment or manpower deployment was not satisfactory. Conclusion: Medical staff are at the core of serving patients. In this study, different occupational categories, age, education, or work experience may influence the incidence of patient safety accidents. However, job satisfaction, management mode and teamwork have a greater impact on personal safety awareness, resulting in a decrease in patient awareness and sensitivity to accidents. It is suggested that management can adjust the work environment, management skills, friendly communication or promotion incentives to maintain the standard of medical care and patient safety.

目的:當今醫療機構首要面對的課題是醫護人員的長期過勞,本研究想瞭解「病人安全態度量表」所測得的病人安全文化風氣推展是否得當;若能同時搭配事後收集的病人安全相關資訊,應能對病人安全風險洞燭先機、提早因應,降低事故發生機率。實驗方式:本研究採用杜克大學賽頓博士所發展的醫院病人安全態度量表為藍本,探究病人安全文化;並以2017至2019年北部某醫學中心之醫護人員為對象。問卷共發出1284份,有效回收問卷為1020份,有效回收率為79.44%。探究病人安全文化在各個面向間之關聯程度,以及分析病人安全文化各個面向,對整體醫護人員滿意度之影響程度。結果:本研究呈現壓力認知與工作狀況之間存在顯著的負相關,說明:當醫護人員壓力越高時,工作狀況越差。另外,在衡量病人安全文化的各個面向中,工作狀況的表現是不理想的,顯示多數的醫護人員認為目前身處的工作環境或人力調遣情況不甚理想。結論:醫護人員身處服務病人核心,若能加強其對病人安全文化的態度,不但能預防各項醫療錯誤的發生,也可提升更具品質的醫療服務。雖然,不同的職業類别、年齡、教育程度或工作經驗會影響病人安全意外事件發生率。但是,工作满意度、管理模式和團隊合作更會影響個人的安全意識,造成病人意外事故的警覺性和敏感性降低。建議管理階層可以調整工作環境、管理技巧、友善溝通或晋升激勵,以维持醫療照護的水準和病人的安全。
