Background: ”Sex” is an important issue in life and sexual relationship is extremely important in interpersonal relationship. Few research related to sexual knowledge among elementary school students has been made. From the limited surveys made before 2001, all had showed that the sexual knowledge among elementary school 5th and 6th graders was generally insufficient. In the present society, the elementary school student's lack of knowledge has caused considerable concern. For improving the sexual knowledge, in addition to the parents and the teachers, the family doctors who provide the holistic care should be the perfect sexual education provider for schoolchildren. This study was conducted to understand how much sexual knowledge did elementary school 5th and 6th graders had. Methods: This study included 151 elementary 5th and 6th graders in one of Taipei elementary school. The questionnaire is based on the literature review of students' sexual knowledge and the sexual education they had received. Results: We found that high percentage of the students had moved to physical maturity well and developed earlier than previous studies, 39.5% and 89.3% among female students had menstruation and breast development respectively, 36.0% among male students had voice change. However, there were only 53 students (35.1%) got the grade above 60% of the total score on the sexual knowledge test. The average score of the female students were higher than the male students. The percentage of the correct answers was much lower for the multiple choice questions than the single choice questions. More than half of the students had reasonable idea concerning ”the gender equality” and ”the social morals of both sexes”. Although the majority of the students were confident of understanding the sexual knowledge they had learned, but actually most of their answers were incorrect and were statistically significant worse than their self perception (p<0.05). Conclusion: High percentage of the elementary school 5th and 6th graders were physically well developed, but most of their sexual knowledge was still lagged behind. The more efficient sexual educational program to improve the sexual knowledge of elementary schoolchildren is needed and must start from the lower graders.
