Predictive Value of Four Parameters in Plain Film for Diagnosis in Children with Intussusception
四種腹部X光之表徵於兒童腸套疊之診斷
摘要
背景和目的：腸套疊在嬰幼兒時期是一種常見而且緊急的疾病，如何能做出快又正確的診斷對一位兒科醫師來說是很重要的。方法：腹部X光上的某些特殊腸氣分布曾被報導過能用來作為腸套疊的診斷工具，所以我們將34位確定診斷為腸套疊及34位確定排除為腸套疊的病童納入本次研究。每位病童的腹部X光片都被專門的放射科醫師隨機的檢視是否有下列四種徵兆：(1)迴腸的擴大；(2)迴腸內空氣的側移；(3)X光片上腫塊的出現;(4)半月板型徵兆的出現。結果與結論：經由各種分析後，我們發現以上四種在病童腹部X光發現的徵兆皆與腸套疊皆有很明顯的相關。因此根據本次研究結論：腹部X光片上的某些特殊腸氣分布是可以當成一種輔助的方法來作為早期兒童腸套疊的診斷與表徵。

Background and Purpose: Intussusception represents the common abdominal emergency in young children. Accurate and rapid diagnosis of intussusception is very important to pediatricians. In early childhood, characteristic gases patternwith the intussusceptionwhich are often visible on plain radiographs which can be used for diagnosis. Methods: Plain abdominal films of 34 children with proved intussusception, and similar number of control cases were reviewed to detect the following 4 parameters: (1) dilatation of the ileum (DI), (2) lateralization of the ileum (LI), (3) soft mass sign (SM), (4) meniscus sign (MS). Results and Conclusion: By using multivariable analysis, it appeared that all the 4 parameters had discriminatory value (P＜0.05). The result indicate that the 4 parameter findings (DI, LI, SM, MS) in plain radiographs can be used as an adjuvant indicator for early diagnosis of intussusception in childhood.
