降低精神科急診病人違規持有危險物品之發生率

Reduce the Rate of Possessing Dangerous Items by Patients with Emergent Psychiatric Admission

摘要


個案醫院為降低精神科急診病人違規持有危險物品之發生率,以減少急診因病人持有危險物品引發的意外事件,如:自殺、破壞、攻擊…故組織品管圈,運用品管圈手法,以增加醫療環境的安全性,提升急診服務品質。自民國92年8月至民國93年4月間,組成急診室品管圈,以腦力激盪、查檢表進行現況分析,發現急診病人違規持有危險物品之發生率達11.24%。經要因驗證得知主要問題為缺乏說明輔助工具、家屬對危險物品的認知與工作人員不一致、家屬知道規則但不配台、缺乏宣導、同一時間來兩個以上病人及工作人員太忙無法顧及等五大要因。依照要因分析,擬定四個對策群組:以輔助工具提升家屬與工作人員對危險物品認知的一致性、提升家屬配合度、增加宣導方法、減少急診病人違規持有危險物品之發生率。實施對策後的效果檢討發現發生率已降為2.93%,且實行期間未出現因急診病人持有危險物品而引發的意外事件。本品管圈之推行成效,顯示成功地以品管圈手法降低精神科急診病人違規持有危險物品之發生率,增加醫療環境的安全性,也提升急診服務品質

The purpose of this study was to decrease the rate of possessing dangerous items of emergency room patients, and to prevent the patients from committing suicide or attacking people with the dangerous items in a psychiatric hospital. The percentage of patients holding dangerous items in the emergency room was found to be 11.24%. A quality control circle (QCC) was formed, and four strategies was used to resolve this problem from August 2003 to April 2004. The rate of patient's possessing dangerous items in the emergency room decreased to 2.93%. In addition, no related accidents occurred in the emergency room during the study period. The study showed that the rate of patient's holding dangerous items in the emergency room was reduced with the QCC approach. The quality control circle process successfully ensured the security of the healthcare environment and the quality of emergency care.(Full text in Chinese)
