臺北市憂鬱症共同照護網教育課程滿意度調查

Satisfaction with the Educational Program of Taipei City Depression Collaborative Care System

摘要


目的:本研究欲評估臺北市憂鬱症共同照護網教育課程的滿意度。方法:選擇民國九十二年至九十三年已經完成憂鬱症核心教育課程之醫療人員,共756位進行滿意度調查。用郵寄方式附回郵信封,兩週內回收問卷。若沒有回信者,經由研究助理用電話說明催促。問卷分析方法以描述性統計分析每個項目的總數與百分比。結果:756份郵寄問卷共回收260份,回復率34%,有效問卷為242份。百分之九十的成員對於核心教育課程的內容表示高度肯定。百分之六十五至八十的成員對於憂鬱症與自殺意念的診斷與處置有中等的信心度。精神科之轉介率(60%)與使用抗憂鬱劑的比率(41.7%)都明顯增加。有百分之七十七的成員對於繼續教育課程的安排覺得適當。結論:憂鬱症教育課程已經達到初步功效,尚須配合更紮實之臨床督導與繼續教育課程加上完整的健保給付與,以便讓基層醫療人員發揮憂鬱症防治第一線尖兵之功效。

Background and Purpose: The purpose of this study is to evaluate professional medical satisfaction with the depression educational program for the Depression Collaborative Care System. Method: We recruited community medical professionals who had trained in the educational program from 2003 to 2004. Seven hundred and fifty six medical professionals received our questionnaire. Study assistants reminded the people who did not respond in two weeks by telephone. We used descriptive statistics to describe each item in the questionnaire, such as totals and percentages. Results: Two hundred and forty two questionnaires were effective. The response rate was 34%. Ninety percent of medical professionals revealed moderate satisfaction with the educational program. Sixty five to eighty percent revealed moderate confidence in the diagnosis and management of depression and suicidal tendencies. A moderate increase in psychiatric transfer rate (60%) and antidepressant usage rate (41.7%) were noted. Most people (77%) were satisfied with continuing educational program. Conclusion: The depression educational program has achieved its initial purpose. If the Bureau of National Health Insurance could reimburse community physicians who take care of depressive patients at the outpatient's clinic, then the collaborative care system for depression would be more effective. In addition to the incentives mentioned above, it is crucial to set up an effective clinical supervision combined with practical continuing education programs in order to equip community medical professionals who can play a more efficient role to prevent depression on the front-line.
