摘要

關節內重建(Intra-articular Reconstruction)是目前膝關節十字韌帶重建手術的主流治療方法，手術主要目的，是重建一條新的前十字韌帶(anterior cruciate ligament, ACL)或後十字韌帶(posterior cruciate ligament, PCL)，以恢復其維持穩定的功能。不論選擇那一種方式都有其優缺點，謹慎選擇適當的材料，並接受正確的手術固定及復健治療，加強肌肉力量與關節彎曲度的訓練，幾乎都能獲得很好的癒後。在傳統上，由先前的打開關節並使用股四頭肌、tensor fascia lata或hamstring轉移重建，不但傷口大，病人復原慢且後遺症大，隨科技進步，現經由關節鏡的使用及器械的改革，可在小傷口、病人恢復快等優勢下，而有顯著的進步。

Intraar ticular ligament reconstruction is a main procedure for severe knee ligament injury. The purpose of reconstruction is to create a new anterior or posterior cruciate ligament (ACL, PCL) for maintaining the knee function. Although, many procedures might be used for reconstruction but some benefits and complications had also been reported with different procedures. So, the careful selection of material, procedure, and rehabilitation program are very important. In traditional reconstruction procedures, the quadriceps tendon, tensor fascia lata, patellar tendon and hamstring tendon had been used as autografts, but the need for opening the knee capsule might cause many complications and discomforts for the patients. And now, the successful use of the arthroscopy or combined arthroscopic procedure for intraarticular ligament reconstruction may minimize the operation wound and enhance early range of motion of the injured knee joint.
