目的:現今的醫療設施與照顧模式大部分是為了處理急性問題來設計,對於慢性疾病問題特別是關係到病人行為改變、心理諮詢與各種專業協同照顧的部分有明顯的不足,許多慢性病人缺乏長期的健康監測,一旦發生急性問題亦無法順利獲得良好的轉介處理,使得慢性疾病控制不良,各種急性併發症增加,進一步佔用寶貴的醫療資源。近年來諸多學者結合感測與通訊技術進行居家健康監測系統的研究或是社區健康照護的服務,可以分擔高成本的醫療人力與資源,居家環境或集合型社區將會是健康與醫療的第一道防線。方法:本系統以微軟所開發的IIS(Internet Information Server)伺服器、PHP與MySQL資料庫程式開發,提供遠距行動照護管理,提出更多元化、行動化之醫療照護服務。結果:本研究應用行動通信基礎與國內醫療院所之醫療資訊系統(HIS)結合,建立一個透過簡訊傳輸生理訊號的遠距居家照護管理系統,並長期且持續性的健康監測。結論:本研究提供了一個遠距居家照護之管理系統,利用無線網路傳輸方法,將生理訊號傳輸至遠距居家照護管理系統,希望能夠為未來相似的研究提供可重複利用的結構,以節省相關媒介建構的時間。
Objective: Most current heath care models and medical facilities are designed for acute problems. These facilities are not as common for chronic disease patients, particularly those related to changes in patient behavior and psychological counseling and those in which a variety of professionals take care of the synergies. Many patients with chronic disease lack long-term health monitoring. Once an acute event occurs, the referral system is inadequate, making chronic disease control ineffective and increasing the variety of complications from medications. This consumes valuable health care resources. In recent years, sensors and communication technology have been combined in building home health care monitoring systems and community health care services. This has been done to reduce the high cost of health care and human resources, with community or home-based health management acting as the first line of defense in the medical service system. Methods: The system was developed based on Microsoft's IIS (Internet Information Server), using PHP (Hypertext Preprocessor) and MySQL database programming to provide management of longdistance home care and more diversified healthcare services. Results: By combining the hospital's information system (HIS) and mobile communication technology, we established a home-based healthcare management system. We expect that the quality of long-term, sustainable healthcare can be improved by monitoring patients’ medical data through the messaging transmission service. Conclusion: Our research provided a home care management system. By using wireless connections through a short message service, patients can collect data at home and transmit it to a central database monitored by healthcare professionals. This system has been adopted in the Mobile Taiwan (M-Taiwan) project and has become a reusable architecture for future Ubiquitous Healthcare (U-health) projects to save time in system construction.
