影響臺灣哺乳婦女捐乳之相關因素探討

摘要


目的:為有效維持早產兒營養及國內部分婦女有感產後母乳分泌不足,期望積極倡導母乳捐贈。臺北市立聯合醫院於2004年12月成立亞洲第一座母乳庫,藉由捐贈母乳使早產兒獲得重要營養支持。方法:本研究旨在深入探討哺乳婦女捐乳動機、家庭支持及對本院提供之捐乳服務滿意之捐乳影響因素。以母乳庫為研究區域,採質性研究設計,以「半結構式深度訪談」方式,依訪談大綱,蒐集資料。結果:助人行善、行有餘力、捐贈餘奶及足夠資訊正向經驗引發動機;寶寶健康泌乳量增、親朋欽羨及哺乳經驗之家庭支持有助持續捐乳;溫馨服務以客為尊捐乳滿意有助強化捐乳行為。結論:藉由捐乳滿足服務品質,提供充足捐乳訊息,與強化哺乳婦女的正向哺乳經驗及家庭支持系統引發捐乳動機,提升哺乳婦女持續捐乳之行為。

Objectives: In Taiwan, the first human milk bank was established at Taipei City Hospital in 2004. To help premature infants who need a continuous supply of human milk, breastfeeding mothers are encouraged to enter the program and donate their surplus milk to the milk bank. The aim of this research is to explore the factors influencing the motivation to donate milk, particularly family support of the practice and donor satisfaction with the quality of service. Methods: This study used qualitative research approach that collected data by in-depth interviews using a semi-structured questionnaire. Results: The factors influencing donor motivation included a good deed that helped others who were in need of help, self-benefit and convincing information. The breast feeding mothers were encouraged by families and friends, had faith in their support of milk donation and continued lactation longer than initially intended in order to donate more milk. The service provided by the hospital service was based on customer-oriented flexible donation time, which was rated as highly satisfactory. Satisfaction with the donation of milk is able to reinforce donation behavior. Conclusion: Ensuring the quality of the service and building awareness among milk donors are the best strategies for motivating breastfeeding mothers to become regular donors.
