Constructing a New Value-Based Surgical Health Care in the Taipei City Hospital

摘要


在柯文哲市長上任後,臺北市立聯合醫院的整體目標有很大的改變。聯合醫院的醫師群,在黃勝堅總院長的帶領之下,走出白色巨塔、進入社區、深入家庭。聯合醫院的醫療服務,也藉由醫院裡的醫病溝通會議、以病人為中心的決策醫療、出院準備服務、居家醫療與專業轉銜服務、安寧溝通會議與安寧觀念的實踐,充分做到陪伴關懷的人文醫療。聯合醫院在黃勝堅總院長指導下,更提出了價值醫療的觀念,期望在有限的醫療資源之下,提供病人最適當、最有效益的全人醫療。聯合醫院外科部據此提出了「建構聯醫外科醫療新價值模式」的論述,建立了「五- 六- 七 價值循環」模式。藉由觀念的改變,實踐「從疾病治療到全家照護、從尖端醫療到社區照顧」的一條龍服務,外科醫師不只是病人最好的醫生朋友,更是病人健康的領航者。

The overall goal of Taipei City Hospital has changed a lot after Dr. Ko Wen-je becomes the mayor of Taipei city. Under the guidance of Dr. Huang Sheng-Jean, Superintendent of Taipei City Hospital, physicians and surgical teams of the Taipei City Hospital go into the community and in-depth to vulnerable families. With the support of family medical communication conference, patient-centered decision-making policy, discharge preparation service and hospice communication meeting, the medical service of Taipei City Hospital fulfills the goals of humanity medicine, accompanying and care. Also, the Taipei City Hospital introduced the concept of value-based care, hoping to provide appropriate and most effective holistic care to the patients. The surgical department of Taipei City hospital raised the thought of "development new value of surgical care" and "the five-six-seven value cycle" model was developed. With the change of concept, the one-stop service of "from the disease treatment to family care, from cutting-edge medical care to the community care", the surgeon is not only the best doctor friend of the patients, but also the navigator of patient healthcare.
