According to the long-term care service planning proposed by the Ministry of Health and Welfare, 70- 80% of the aging population requiring long-term care service may need community-based or home-base care. Home-based care has been promoted in the Taipei City Hospital since 2016 in which the occupational therapists extended their services from hospital to community. Current occupational therapy services in the Taipei City Hospital include home-based rehabilitation and palliative care . In this article, we would review the literature on home-based occupational therapy care. Furthermore, our hand-on experiences would be introduced. The purpose of this article is to provide new insights from the view of occupational therapists for those embark on similar endeavors.
