職能治療居家訪視在腦中風急性後期照護之經驗分享

Experience Sharing on Home Visit Occupational Therapy in Post-Acute Stroke Care

摘要

腦中風照顧是一連續性的照護，其中包括急性住院的病況控制、出院前之準備及出院後居家照護的轉銜過程。職能治療除了出院準備一般常態性服務外，更提供居家環境評估、輔具資訊、復能活動等相關諮詢與建議。本文介紹台北市立聯合醫院職能治療師到腦中風急性後期照護個案家中執行居家環境評估，給予環境改善（94.7%）、輔具資訊（91.5%）及復能活動（93.1%）建議。職能治療組從2017年12月至2020年11月執行上述服務共計319人次。在居家環境中最常建議項目是浴室的防滑措施、加裝扶手與行動路線改善，在輔具方面最多建議是購買拐杖、沐浴椅、輪椅及便盆椅等。居家訪視的重要性是讓個案在出院後能無縫接軌居家無障礙環境生活，使個案發揮最大潛能，同時也減少照顧者的負擔。建議可以將職能治療居家訪視納入腦中風出院準備服務中，讓病人與家屬獲得更多的助益。

Stroke care is continuous care series including control of acute hospitalization, discharge preparation, and transference for home care. Besides regular discharge service, occupational therapy offers consultations and recommendations for the home environment, assistive devices, and reablement. This article describes the home visit experiences of post-acute care-cerebrovascular diseases (PAC-CVD) patients of the occupational therapists of the Taipei City Hospital. A total of 319 visits were carried out from December 2017 to November 2020. They offered suggestions on environment modifications (94.7%), assistive devices (91.5%), and reablement (93.1%). The most common suggestions were anti-slipped measures in the bathroom, handrails installation, and improvement of movement routes. Crutches, bathing chairs, wheelchairs, and potty chairs were the most common suggested assistive devices. The importance of home visits is to enable the patients to connect to a barrier-free home living environment seamlessly to maximize the patient's capability and reduce caregivers' burdens. We recommend incorporating occupational therapist home visits in discharge preparation service to offer more benefits to the patients and their families.
