Decreased Outpatient Visits in Patients with Asthma or Allergic Rhinitis Receiving Herbal Patching in Sanfu Days

摘要

目的：本研究欲分析中醫三伏貼與氣喘、過敏性鼻炎醫療利用的相關性，觀察其是否降低過敏性鼻炎或氣喘患者的門診就醫次數的療效。方法：採用差異評估法（Difference-in-Difference，簡稱DID）比較2011年有使用三伏貼的實驗組與未用三伏貼的對照組，其前三年(2008-2010年)與後三年(2012-2014年)的過敏性鼻炎及氣喘門診的就醫紀錄之差異。結果：從2008至2014年間總共有70529人就診，其中僅2198人接受三伏貼，佔3.12%，其中有1941名過敏性鼻炎患者，257名氣喘患者；各年度就醫人數以女性居多，使用最多的年齡層為5~14歲；配對比較在2011年有使用三伏貼的過敏性鼻炎、氣喘患者，後三年平均門診就醫次數均減少，過敏性鼻炎患者後三年的平均門診就醫次數減少2.45次，氣喘患者則減少2.98次，反之對照組的平均門診就醫次數均有增加。結論：本研究發現三伏貼療法可降低過敏性鼻炎及氣喘的門診醫療資源耗用次數，以此提供另一種觀點，作為未來相關研究的參考。

Objective: This study aimed to investigate the treatment efficacy of acupoints herbal patchingin Sanfu Days in asthma or allergic rhinitis by evaluating the frequency of outpatients visiting of the patients. Methods: Difference-in-Difference (DID) method was used to evaluate the frequency of outpatient visit between the experiment group who usedacupoints herbal patching in 2011 and the control group who did not used the patching in 2011, three years before (A.D. 2008-2010) and three years later (A.D. 2012- 2014). Results: A total of 70,529 patients visited the outpatient clinics for allergic rhinitis and asthma between 2008 and 2014. Among them, 2,198 patients received acupoints herbal patching (3.12%), including 1941 patients with allergic rhinitis and 257 patients with asthma. The predominant gender was female and age group was 5 to 14 years. The average frequency of outpatient visitsof the patients who used acupoints herbal patching decreased three years later:the frequency of outpatient visits of patients with allergic rhinitis decreased 2.45 times whilethe frequency of outpatient visits of patients with asthmadecreased 2.98 times. On the contrary, the frequency of outpatient visits of patients in the control groupincreased. Conclusion: Acupoints herbal patching in Sanfu Days decreased health service utilization of the patients with asthma and allergic rhinitis. This finding provides an additional optionfor the evaluation of the efficacy of acupoints herbal patchingin Sanfu Days and further investigation is warranted.
