摘要

安寧家屬的悲傷從病人死亡起邁入另一個階段。悲傷處遇需要持續地介入與關懷，協助家屬接納親人逝去的失落。然而，現行許多悲傷輔導方式仍侷限於追思會或電話關懷，因此我們期望能發展出較為完整的悲傷關懷流程，建立一個創新的悲傷輔導模式。本文將介紹本院發展之後期悲傷輔導模式。除喪親家屬關懷追蹤服務外，也藉由情緒支持團體，運用自然、藝術、音樂等元素，引導喪親家屬面對與調適悲傷情緒，發展同儕支持力量，協助有悲傷輔導需求的家屬處理悲傷失落議題。從團體成員及專業人員給予的回饋中我們發現下列成效：提供喪親者同儕支持的力量、提供家人互相支持的力量、提供一個自然溫馨的場域讓安寧團隊可持續關懷喪親者、照顧團隊注入新的能量。

The sadness of palliative family members transfers to another stage from the day the patient died. Bereavement care should be a continuous intervention and care to help families accept the loss of their loved ones. However, currently most bereavement care is limited to memorial or telephone follow-up. This study aimed to develop a more comprehensive grief counseling process by establishing an innovative bereavement care model. This paper introduces a grief counseling model developed by our team. Apart from ordinary bereavement care for the family members, we have created an emotional support group that use nature, art and music elements to help the bereaved families to face and deal with grief. The support group helps develop peer supports and supports families in need to cope with the issue of sadness and lost. According to the feedback of the group members and the reflections of our professional team, this new model was successful in providing a strength for peer supports to the bereaved and between family members, a natural and warm context for the palliative care team to continuously care for the bereaved, and positive rewards to the care team.
