摘要

以人為中心的失智症整合照護用於支持失智者及其家屬，期待以此模式延緩失智進程，增強失智者的被照護經驗以及改善社會照護。北市聯合醫院整合7院區的失智照護資源，透過跨專業團隊合作模式，組成聯醫失智症照護聯盟，照護台北市12個行政區失智者及其家庭。期待失智者及其家庭在此整合模式下，於社區中生活時可同時獲得尊嚴、尊重、自主、平等。透過與民間單位和政府機關合作，運用創新科技，期待由醫療出發，深耕社區，為失智者提供全方位照護，創造失智友善的臺北。

Person Centered integrated care for dementia is used to support dementia patients and their families, and it is expected that this care model will delay the process of dementia, enhance the care experience of demented people, and improve social care. The Taipei City Hospital integrates 7 dementia care resources to form a joint medical dementia care alliance through an interprofessional practice model to care for the persons with dementia and their families across 12 administrative districts in Taipei. It is expected that the persons with dementia and their families will benefit from dignity, respect, autonomy, and equality in the community when living in the integrated model. Through cooperation with non-governmental organization and government agencies, we use innovative technology and look forward to start-off from medical care, deepen into the community, provide comprehensive care for dementia, and create a dementia friendly Taipei city.
