推動機構式安寧照護

2015年臺北市立聯合醫院松德院區開始推動社區安寧，因精神專科之特殊屬性，以發展「機構式安寧照護」為服務模式。透過安養機構住民屬性調查、機構安寧推動困難分析，以規劃、執行、確認及精進等四階段實施。實施以來，收案119位安寧照護住民，非癌症比率佔91%，個案在機構往生比率由15%提升至36.9%。召開150場安寧緩和家庭會議，20場次安寧座談及宣導，提升住民、家屬之死亡識能。此外，機構安寧也獲得正向回饋，家屬滿意度達97%，機構滿意度100%，2017台灣醫療科技展將機構安寧列入照顧模式，機構安寧躍居醫療產業，且安寧療護於跨專業實踐具可行性，期許臺北市立聯合醫院松德院區之推動經驗，成為精神專科醫院與安養機構合作之示範。

Since 2015, our hospital began to promote community hospice. We developed a service model of Palliative Care in Nursing Home because of the psychiatric hospitals' unique attributes. Through the survey of resident's properties and the analysis of difficulties with promoting hospice in the nursing home, we have developed a four-stage implementation plan, namely, planning, implementation, confirmation, and advance. Since the implementation, 119 hospice residents have been taken care of, among which 91% were non-cancer; the passing rate in the nursing home has also increased from 15% to 36.9%. A total of 150 palliative family conferences and 20 hospice communication with the residents and families were held to improve the death literacy. Furthermore, the Palliative Care in Nursing Home also received positive feedback of 97% satisfaction from the families and 100% satisfaction from the facilities. The 2017 Taiwan Healthcare Expo has listed Palliative Care in Nursing Home as a care model and has become part of the medical industry. As a result of the COVID-19 epidemic in 2020, more family members are willing to consider terminal life choices for the elders. Palliative Care practice has the feasibility in transdisciplinary care. We expect our promotion experiences to become a demo for cooperation between psychiatric hospitals and nursing home.
