摘要

目的：臺北市立聯合醫院配合臺北市政府衛生局政策，自104年起推動「醫院社區整合居家藥事照護」，本文回顧本創新服務推行歷程及成效分析。方法：建立「醫院社區整合藥事照護」服務模式，並建置「藥事照護資訊系統」提供訪視藥師使用，針對病人滿意度、再訪需求性、藥事照護提供之用藥指導項目、病人之用藥問題等進行統計分析。結果：自105年至108年共提供整合藥事照護服務14,349人次，研究期間87.9%的訪視對象對本計畫之服務表示滿意，51.9%病人有藥師再次到宅提供居家藥事照護之需求。結論：臺北市「醫院社區整合藥事照護」提供高齡、獨居失能病人安全用藥照護品質，執行成效廣為各界肯定及好評。推動經驗期以公益協助全國推行，造福社會更多弱勢病人。

Objective: In coordinating policies of the Taipei City Government Health Bureau, the Taipei City Hospital has promoted the 〞Hospital-Community Integrated Pharmaceutical Care〞 since 2015. This article reviews the implementation process and analyzes the effectiveness of the innovative service. Methods: We established a 〞hospital-community integrated pharmaceutical care〞 service model and a 〞pharmaceutical care information system〞 for pharmacists to record patient satisfaction, revisit needs, medication guidance/recommendations provided by pharmacists, and medication problems, and conducted the statistical analysis. Results: A total of 14,349 integrated medical care services were provided from 2016 to 2019. During the research period, 87.9% of the patients expressed satisfaction with the services, and 51.9% needed pharmacist revisit. Conclusion: 〞Hospital and Community Integrated Pharmaceutical Care〞 provides safe medication care for elderly and disabled patients living alone in Taipei. The effectiveness is affirmed broadly and ranked positively. We expect and hope to help promote the service nationally to benefit more minority patients.
