Objective: Very few reports have described the cytological features of endometrial clear cell carcinoma (CCC). We report the cytomorphologic features of a case of endometrial CCC. Case report: A 59-year-old woman with no major past medical history participated in the routine cervical cancer screening in our hospital. The cervical cytology showed clusters of pleomorphic adenocarcinoma cells with high nuclear/cytoplasmic (N/C) ratio, vacuolated cytoplasm, distinct cell borders, round to oval centrally located nuclei, large nucleoli, and fine granular chromatins. Granular diathesis in the background was noted. Endometrial curettage revealed a picture of CCC. Conclusion: To be familiar with the presentation of cytologic features of endometrial CCC is helpful for earlier diagnosis before the symptoms and signs develop.
