從以人為本理念建置新冠肺炎之護理整備作為－以臺北市立聯合醫院為例

摘要

2020年面對猛烈的新冠肺炎疫情，臺灣醫療體系迅速動員，制定各項防疫措施，及早辨識病例、本土境外圍堵隔離、資源合理分配及管控，並整合健保、海關移民等資料進行大數據分析，建構完善指揮架構，加上疫情資訊公開透明，是穩固疫情之重要措施。北市聯醫團隊秉持以人為本與人文關懷的核心價值，在嚴謹的檢疫流程與醫療照護作業，以病人為中心、團隊為基準，提供有效、全面、可近及連續性的照護模式。各院區護理主管透過溝通合作、共同分工負起重責，啟動擬定防疫策略及作為，如：護理人力資源調配機制、專責病房重整規劃、防疫教育訓練、發展臨床防疫作業標準及監控（如：COVID-19疑似個案，手術全期、生產、洗腎、呼吸器使用等）及多樣性防疫創新作為等。護理部門配合國家、醫院政策計畫並依據現況做可行性分析，針對人員量能準備、以人為本服務、管理制度重整、防疫物資存備管理、硬體環境佈置等五大精進防疫方針做佈建整備計畫，以減少醫療資源浪費及提升防疫品質。

Facing the outbreak of COVID-19 in 2020, Taiwan Healthcare System quickly developed various strategies to prevent the epidemic, including early cases identification, screening technologies, quarantine rules, resources allocation, public health restriction, and integration among the national health insurance, immigration and traveling datasets for TOCC checking. Taiwan also established the central conduct authority, shared transparency, and publish the epidemic information. The above are all important interventions for the outbreak control. Medical care teams in Taipei City Hospital are based on the core value of the people-centered care and humanistic compassion, under the strict planning of the quarantine process and medical regulations, on guard without fear but positive attitude to face this severe challenge. In each branch hospital, they provided the epidemic prevention strategy, and as the head of nursing department , we had more responsibilities, including the strategy implementation program as nursing human resources deployment mechanism, units reorganization planning, multiple education and training in epidemic prevention, to establish the operating standards ,monitoring of clinical epidemic prevention (e.g. SOP to monitor COVID-19 suspected case, hemodialysis, ventilator operation, and etc.), and innovations about epidemic prevention. According to the national and hospital policies and the analysis of current situation, the Department of Nursing should be combat by leading five major directions, including the capacity and preparation of nursing personnel, providing people-centered services, management of system restructure, controlling epidemic materials storage, and rebuilding of environment construction.
