運用專案改善提升手術室組織檢體運送完整性

Using Project Management to Promote Security of Pathological in the Operating Room

摘要


組織檢體的收集及運送是醫療過程中的重要項目,任何一件組織檢體的運送過程中發生錯誤時,不但會造成患者疾病診斷與治療受到不可彌補的錯誤,也會增加醫院成本浪費及人力的負荷。本專案是以某區域教學醫院為例,自2009年4月至8月期間送檢總數為1692件,有8件發生錯誤,錯誤率為0.47%。分析原因有缺乏送檢單、病患基本資料錯誤及檢體數量錯誤等問題,擬執行修改「手術室組織檢體送檢流程」、修改新制檢體簽收單張、規劃冰箱硬體擺設、增添設備(防毀貼紙、封口機器、上鎖收集盒及電腦)之方案,並強調檢體收集及保存的重要性,甚至嚴防檢體被調包之風險性,自2009年9月至10月執行改善措施,於2010 年3月檢體錯誤已下降為0件,檢體送檢完整率達到100%。

The specimen collection and delivery is one of the important medical procedures in the operating room. Any mistakes during the process may lead to misdiagnosis and inappropriate treatment, which in turn will increase staff workload and hospital costs. This project was conducted in a regional teaching hospital investigating the errors happened during collecting and delivering the specimens. From April to August 2009, 8 errors out of 1692 cases (0.47%) were reported. The errors were resulted from lack of lab forms, forms with incomplete info, and incorrect count of specimens. The project management initiated several strategies for improvement. These included establishing the standard procedures for specimen collection and delivery, e.g., checklist, storage, transporting lockers, antifraud stickers, sealing devices, and computerizing bar-code system. These actions strengthen the security of specimens and prevent from the risk of inappropriate switching the specimens. After the project management was introduced, the mistaken rate decreased to zero and the goal of secure specimen delivery can be achieved. This project increases the work efficiency in the operating room.
