降低外科臨床護理人員逾時下班之改善專案

Improvement of Nursing Work Overtime in Surgical Ward

摘要


護理人力不足是目前國內醫療院所面臨棘手問題,目前多以加班或逾時因應。本專案自2010年4月1日至2010年8月31日止,發現單位逾時原因為照護病人數多且出入院變動大、交班內容重複抄寫且交班時間長、護理人員需親自推床接刀等,而在護理部人員關懷小組進行關懷輔導時亦得知離職原因大多為工作屬性過於複雜,加上病人出入院頻繁,病情複雜度及嚴重度高,也因此造成護理人員常有逾時情形,導致護理人員出現疲潰感,相對的影響其照護品質,故藉由降低照護病人數並減少組內流動率、修正交班模式、訂定接刀準則等措施解決造成逾時下班之問題。結果:護理人員逾時率由85.7%降低至60%。期望未來能針對逾時原因各細項作深入探討,以作為臨床業務及管理參考。

The shortage of nursing professionals is a common problem in Taiwan, impacting patient safety and nursing quality. This project was designed to reduce the amount of working hours per nurse. The setting of the study was a regional teaching hospital in southern Taiwan. Data was gathered from 2010/04 to 2010/08. The head nurse used the "time-out survey work sheet" to record patient personal information, working hours of nursing activity for every nurse shift, and used the human resources system to verify the timeout. A Standard Protocol and hand-over shift change was created, which showed that overtime rate decreased from 85.7% to 60%.
