老年2型糖尿病婦女面對生活與疾病經驗

A Qualitative Study of Confronting Life with Illness Experience in Elderly Women with Type 2 Diabetes Mellitus

摘要


目的:透過質性方法描述台灣老年2型糖尿病婦女的疾病控制經驗。方法:使用詮釋現象學的質性研究法,15名年齡在62-75歲的台灣女性描述他們的生活和應對2型糖尿病的經歷。研究方法是採半結構式指引進行深度訪談,使用主題分析法進行分析。結果:經質性分析歸納為四大主題:1.心理和情感困境,包括與疾病共存的家庭緊張局勢,感到無力,缺乏信心;不安全感包括低血糖和糖尿病併發症的恐懼。2.對治療相關訊息的誤解,包括對血糖值缺乏了解,和沒有達到目標的自我效能感及胰島素融入生活的困境。3.疾病控制信念低下,包括受制於身體功能退化及受限生活中的既有習慣。4.與家人共同生活上所帶來的挑戰,包括家人無法配合糖尿病飲食及飲食控制處於自主與依賴之間。本研究發現,可供醫療保健提供者鼓勵家庭成員共同參與糖尿病控制,期望本研究結果能作為日後臨床照護之參考。

Objective:Describe the disease control experience of elderly women with type 2 diabetes in Taiwan through a qualitative approach. Methods:Using a qualitative approach to interpretative phenomenology, 15 Taiwanese women aged 62-75 years described their lives and their experiences with type 2 diabetes. The research method is to conduct in-depth interviews with semi-structured guidelines and analyze them using subject analysis. Results: The four major themes: Theme I: mental and emotional problems, including family tensions coexisting with the disease, feeling powerless, and lacking confidence; insecurity including fear of hypoglycemia and diabetic complications. Theme II: misunderstandings regarding treatment-related information, including lacking knowledge about blood glucose level, and having no self-efficacy to attain goals. Insulin is difficult to integrate into life. Theme III: lack of disease control beliefs, including subject to physical control of the body, can not control the disease process; limited living habits, can not take the correct life adaptation strategy. Theme IV: the challenges of living with family members include the inability of family members to cope with diabetes diet and diet control between autonomy and dependence. This study found that health care providers can encourage family members to participate in diabetes control, and hope that the results of this study can be used as a reference for future clinical care.
