過敏性鼻炎患者體質證型之分析

本文是利用多元尺度法(multidimensional scaling)針對過敏性鼻炎患者的體質證型建立一個簡易的體質分型概念。資料是由中國醫藥大學附設醫院所收集107位過敏性鼻炎患者的中醫問診的自覺症狀、發病時段、誘發因素及西醫實驗室檢驗的過敏原篩檢等4 大類的資料，總共有35個變數。由中醫問診的自覺症狀可將此病患者的體質證型分成寒性體質、熱性體質及寒熱夾雜體質等三種，當患者有困倦、怕風寒、肢冷等症狀一種以上出現者為寒性體質，當患者有口渴、口苦、口臭及咽乾等症狀一種以上出現者為熱性體質，而同時含有寒性及熱性體質特徵之症狀者則歸為寒熱夾雜體質。在結合中、西醫學合診的觀點而建立出簡易的體質分型法：遇特殊刺激物而誘發者為熱性體質；遇冷風而誘發、疲勞而誘發或睡眠不足而誘發者為寒性體質；而寒性及熱性現象均有者則歸屬寒熱夾雜體質。簡易的體質分型結果與中醫基礎理論不相違背，而且簡易體質分型結果與傳統中醫問診的自覺症狀分型結果的一致性達85%。

In this article, we use multidimensional scaling method to set up a simple classification rule, and the purpose is to classify the allergic rhinits patients' constitution into three patterns; they are cold pattern, heat pattern and cold-heat complex pattern. Data were collected from Chinese Medical College's hospital and it contains four main categories symptoms, eruption types, stimuli factors and laboratory allergic screen test results. The total has 35 variables. The result is that we can classify a patient's constitution into the cold pattern if allergic rhinits erupt in the morning or caused by cold wind tried or unquit sleep. A patient's physical is heat pattern if allergic rhinits was caused by stimuli factors. If a patient's symptoms has both cold pattern and heat pattern then we treat it as clod-heat complex. The results does not contradict with the fundamental theory of the traditional Chinese medicine. Our classification results consistent with the classification results by using traditional methods.
