末期癌症之死亡恐懼：一個案報告
Fear of Death in Terminal Cancer: Case Report
摘要
本文係探討一位四十七歲末期頰黏膜麟狀上皮癌的患者與家屬在住院期間所面臨的身心靈問題及其護理，以直接照護、觀察、筆談、家屬會談、傾聽及團隊會議探討等技巧收集資料，進一步評估並分析個案，應用護理過程分析及護理計劃，藉由團隊集思廣義，取得彼此合作模式及聯繫，共同協助減輕或解決其疼痛、腫瘤傷口照顧、家屬體力心力負荷過重、心願未了（回家）及恐懼等需求，在家族與醫療團隊支持與努力下，整合引導出病人深層之身心靈困擾與釋放，及時表達道歉與感謝，妻子亦能原諒與接納之照護過程。整合跨團隊的安寧療護提供合乎病人需求的照護方式，竭盡所能給予醫療與護理照顧，使病人症狀獲得有效的控制，心理靈性的困擾獲得釋放，同時體恤並協助家屬能面對與因應病程改變及哀傷！末期病人度過最後一段安適、有意義及有品質的生活，家屬無憾，病人安然走完人生。藉此個案之安寧療護護理經驗，提供日後照顧此類病人的醫療團隊參考。

The purpose of this paper is to share a successful nursing experience involving a hospitalized, 47 year-old buccal cancer male patient who suffered a fear of death from the terminal disease. For advanced investigating this case, we used direct nursing care, observation, communication by writing and family interview etc. We alleviated the symptom of pain, cared the wound of tumor, eased off the family's burden, accompanied the wish of the patient (to return home), and diminished the feeling of fear. We liberated the patients deep baffle: including offering an apology to his family and receiving his wife's forgiven. This successful nursing experience offers nursing staffs to utilize and develop the nursing care policies which promote higher standards of caring, so that those dying patients and their family members would receive better quality of life.
