護理之家護理人員對安寧療護知識及態度的探討

Knowledge and Attitudes Towards Hospice Care Among Nursing Home Nurses

摘要


背景:安寧療護的宗旨是以高人性化的態度照顧生命末期的個案,使其有尊嚴的離世。由於長照機構住民多為高齡或疾病存活期較長的個案,故使用安寧療護的機會也隨之增加。但目前針對照護人員安寧療護知能的相關研究,大部份是以醫院的照護人員為主,而對長照機構照護人員的研究仍較少見。研究目的:本研究探討護理之家護理人員對安寧療護知識與態度的現況及相關性。材料與方法:採橫斷式設計及叢聚隨機取樣,運用結構式問卷調查全國北、中、南三區護理之家護理人員,最後回收有效問卷共232份,回收率為62%。結果:受訪者平均年齡35.2歲,82.5%的人具有正確的安寧療護知識及正向的態度。教育程度、護理之家型態、工作年資在安寧療護知識呈顯著的差異;而教育程度、宗教信仰在安寧療護態度有顯著的差異。整體來說,安寧療護知識和態度呈正相關,即安寧療護知識愈佳,其態度愈正向。結論:本研究結果能提供長照機構主管瞭解護理人員在安寧療護知識及態度之現況,並鼓勵爲護理人員設計安寧療護教育相關訓練課程,進而提升末期疾病住民的照護品質。

Background:The philosophy of hospice is to assist people in their last stage of life experiencing high humanity and dying with dignity. Nursing home residents tend to be more with advanced age and longer disease-related survivors, thus their likelihood of using hospice is high. However, most hospice studies are conducted in hospital health personnel; little is known about the hospice among nursing home nurses. Purpose:This study was to examine nursing home nurses' knowledge and attitudes towards hospice. Methods: This was a cross-sectional study with cluster sampling. Structured questionnaires were used to collect data. A total of 232 nurses in the certified northern, central, and southern districts of nursing home nationwide participated in this study. The response rate was 62%. Results:The mean age for the sample was 35.2 (SD=8.98), ranging from 22 to 62 years. 82.5 % of respondents showed correct knowledge, and the majority displayed positive attitudes about hospice. Education, nursing home type, and working years indicated significant differences in hospice knowledge. Education and religion were significantly different in hospice attitude. Overall, hospice knowledge was positively correlated with attitude. Conclusions:This study may provide nursing home administrators with a better insight about nurses' knowledge and attitudes regarding to hospice. Considering the need of hospice in long-term care settings, providing nurses with educational programs is of importance, in order to improve quality care at the end of life or for chronically sick residents.
