Learning from practical cases: An effective training program for palliative care social workers

安寧緩和社會工作者的培訓:探究以案例討論為導向的學習成效

This paper examines the effectiveness of a case-discussion oriented training program for social workers who are willing to provide palliative care services to the elderly, the disabled, and patients with life-threatening or terminal illness and their families in communities. The main purposes of the training program are to encourage social workers to participate in the training by shortening training time and utilizing practical cases as training materials to improve training effectiveness. The materials used in the training focus on the cases with three different stages of illness. Trainees use these materials to learn proper actions to improve clients' and their families' well-being. Moreover, the effectiveness of the training program is evaluated through a questionnaire with a total of 45 questions that cover the aspects of knowledge, attitudes, and practice. A statically significant difference is found between the pretest and posttest results of 141 trainees on 13 attitude-related questions, as well as on five knowledge- and practice-related questions. These results indicate that trainees' attitudes, knowledge and practice have all changed, and the changes in the attitudes toward palliative care are most significant after participating in the case-discussion oriented training program. This paper then argues that palliative care training programs should provide trainees with continuing education and opportunities to exchange work experience from different work fields. Through such training programs, social workers can have a better, holistic understanding of their attitudes toward "a good death", and develop reflective thinking skills. These findings would serve as reference for continuing education and future guidelines for palliative care training.

本研究是在安寧緩和社會工作者的培訓計畫中,探究以案例討論為導向的學習成效。研究目的是藉由為讓大部分社區中接觸長者、身心障礙者、罹患生命威脅疾病的病人或家屬的社會工作者,能夠更容易參與訓練課程,以案例討論為導向的訓練,設計三個疾病階段的個案情境進行訓練。課程使用知識、態度、及實務向度各15題的問卷(KAP)做為評值,以測量課程成效。研究結果顯示141位受訓參與者的課程前後測中,在態度面向有13項達顯著差異,知識及實務操作面向則各有5題有顯著差異。發現對以案例討論為導向的訓練模式,參與者態度、知識及實務皆有部分改變,其中特別是態度影響最大。本文提出對社會工作者面對善終的個案時,需要透過持續的教育或不同領域工作的經驗交流,發展他們對善終態度的覺察與統整,建構反思的觀點,做為持續教育與未來指引的參考。
