影像重建方法對Tc-99m Trodat-1腦部單光子斷層掃描特異性結合比的影響

The Influence of Image Reconstruction Methods on Tc-99m Trodat-1 Brain SPECT Specific Binding Ratios

摘要


Tc-99m Trodat-1腦部單光子斷層掃描是用來診斷帕金森氏症(Parkinson's Disease)的利器,特異性結合比(Specific binding ratio, SBR)用來評估腦部紋狀體的藥物吸收,研究大多討論圈選紋狀體(Striatum)的方式對特異性結合比的影響,但對於影像重組參數對特異性結合比的影響較少著墨,本研究就影像重組參數對特異性結合比的影響加以探討。本研究採取回溯性方式,隨機選取36名Tc-99m Trodat-1腦部單光子斷層掃描病人影像,分別以濾波背投影(Metz filter, point spread:9, order:5)及疊代法(OSEM, subsets:10, iterations:2)的影像重建方法,使用PMOD軟體,於濾波背投影重建影像上圈選紋狀體及枕葉之VOI(Volume of interest),並套用在疊代法重建影像上,以計算兩者的特異性結合比。濾波背投影重建影像及疊代法重建影像計算出之特異性結合比有極高之相關性(R=0.866),但疊代法重建影像計算出之特異性結合比數值約為濾波背投影重建影像的2倍(mean=2.14, SD=0.33)。由此可見,影像重建方法對於特異性結合比的數值有顯著之影響,在計算特異性結合比時,應固定影像重建方法及參數。

Tc-99m Trodat-1 brain SPECT is a valid method to evaluate dopaminergic neurons activity. Specific binding ratio (SBR) was used to estimate the striatum uptake. Several studies had reported the methods of drawing the region of interest (ROI) at striatum. However, the lack of study discussed on the effect of image reconstruction parameters on SBR analysis. This study aims to evaluate how reconstruction methods influence on SBR.Thirty-six patients underwent Tc-99m trodat-1 Brain SPECT were enrolled retrospectively. Images were acquired using GE Hawkeye Infinia with fambeam collimators. Images were reconstructed by filtered back projection (FBP) with Metz filter (point spread: 9, order: 5 ) and iterative reconstruction (OSEM) with 10 subsets and 2 iterations. Chang's attenuation correction with coefficient of 0.12 cm-1 was applied. Striatum volumes of interest (VOIs) were draw using PMOD software from FBP reconstructed images. Generated VOIs were applied on both FBP and OSEM reconstructed images to calculate SBRs. The SBR values from FBP reconstructed images showed high correlation with those from OSEM reconstructed images (R=0.866). However, the SBRs values calculated from FBP reconstructed images were higher than those calculated from OSEM reconstructed images by a factor of 2 (mean=2.14, SD=0.33).SBR values can be markedly influenced by image reconstruction algorithms. Image reconstruction parameters should be standardized for SBRs calculation.
