醫事放射師為疑似新冠肺炎患者執行X光檢查之感染安全防護─台灣彰化員林基督教醫院經驗探討

摘要


新冠肺炎(Coronavirus Disease, COVID-19)於2019年12月底始於中國大陸武漢省,疫情擴展迅速,很快就被世界衛生組織(World Health Organization, WHO)證實為國際間重大傳染疾病之一,其強大的傳染力相繼造成世界各國重大疫情。台灣因嚴重急性呼吸道綜合症(Severe acute respiratory syndrome, SARS)的抗疫經驗,不敢輕忽此傳染性疾病。由政府統一分配醫療物資外,竭盡全力防疫社區及院內感染,並對疑似症狀之國際旅客、返國國民及其他高風險族群進行檢疫,指示全台醫療院所對防疫進行佈署。即便如此,全國醫療物資提供各醫療院所,所需之醫護人員個人防疫裝備仍不足,在疫情尚未減緩之時,醫療院所僅能透過疫情分析來阻隔疑似病患,將疑似病患,限制其移動範圍,避免其他來院患者與之交叉感染。本文分享以台灣彰化員林基督教醫院放射科為例,對疑似新冠肺炎患者,集中至急診就診,於發燒篩檢站採檢鼻咽喉、下呼吸道等處檢體後,至隔離區執行胸部X光檢查,確認肺部是否有浸潤肺炎現象。在無完整醫療防疫物資下,醫事放射師利用現有的防疫資源,完成個人防護裝備(Personal Protective Equipment; PPE),以應對受檢之疑似新冠肺炎患者,強化醫事放射師個人防護,避免疑似新冠肺炎患者至X光攝影室後,造成院內感染的防疫破口。

Coronavirus Disease (COVID-19) began in Wuhan, China at the end of December 2019. The epidemic spread rapidly at that time, and was soon confirmed by the World Health Organization (WHO) as one of the major infectious diseases in the world. Its powerful infectious power has caused major epidemics in various countries and regions in the world. Because of its anti-epidemic experience with severe acute respiratory syndrome (SARS), Taiwan dare not ignore this infectious disease. Therefore, apart from the uniform distribution of medical materials by the government, all of Taiwan makes every effort to prevent community infections and health care-associated infections. Quarantine will be conducted against international passengers, returning nationals and other high-risk groups with suspected symptoms. This measure has directed medical institutions across Taiwan to deploy anti-epidemic measures. Even so, in order to provide medical institutions across the country, the personal prevention equipment of the medical staff required is still insufficient. When the epidemic situation has not been alleviated, medical institutions can only block patients suspected of being infected through epidemic analysis. The medical institution will suspect the patient and limit its range of movement to avoid cross-infection with other patients who come to the hospital. This study uses the Radiology Department of Changhua Yuanlin Christian Hospital as an example. For patients suspected of being infected with COVID-19, the hospital concentrated patients to the emergency department. The patient needs to go to the fever screening station first, collect the nasopharynx, lower respiratory tract and other specimens, and then perform chest X-ray examination in the isolation area to confirm whether the lungs have infiltrated pneumonia. Without complete medical epidemic prevention materials, medical radiologists must use the existing epidemic prevention resources to complete Personal Protective Equipment (PPE) to deal with patients suspected of having new coronary pneumonia. Thereby, the personal protection of medical radiologists is strengthened to prevent patients with suspected infection from becoming an epidemic prevention breach that causes health care-associated infection after they go to the X-ray room.
