以乳房攝影定位進行真空輔助切片(SVAB)使用乳房電腦斷層(DBT)進行輔助下探討

摘要


近年來,乳癌逐漸成為全球發展中國家的重要病因之一。罹患乳癌之人數有逐年加速上升的趨勢,依據台灣衛福部國民健康署公佈之2014年癌症登記報告就指出,乳癌於女性之中,仍高居為癌症之首。乳癌已造成女性相當大的威脅,而已知高達百分之五十的乳癌通常由微鈣化形成,針對乳房病變(微鈣化),開放性手術切片(OSB)是標準診斷方式,而現今立體定位真空輔助切片(SVAB)逐漸在近年成為新診斷方式,立體定位真空輔助切片(SVAB)是可靠的診斷工具。以SVAB取代OSB,病患不用住院,切片時間短,併發症也比較輕微,還可以讓良性病變的病患省去不必要的手術。病變(微鈣化)在乳房的深度位置是在進行SVAB的重要依據。本研究在探討,檢查過程中使用乳房電腦斷層攝影技術能準確知道微鈣化(病灶)位置並確定深度,好讓醫師在切片檢查術中進行調整,以達到更多病理組織切片。

In recent years, breast cancer has gradually become one of the important causes of developing countries in the world. The number of people suffering from breast cancer is increasing year by year. According to the 2014 Cancer Registration Report published by the National Health Administration of the Ministry of Health and Welfare of Taiwan, breast cancer still ranks first among women. Breast cancer has caused a considerable threat to women, and it is known that up to 50% of breast cancers are usually formed by microcalcification. For breast lesions (microcalcification), open surgical biopsy (OSB) is the standard diagnostic method, and nowadays stereotactic vacuum Assisted slice (SVAB) has gradually become a new diagnostic method in recent years, and stereotactic vacuum assisted slice (SVAB) is a reliable diagnostic tool. By replacing OSB with SVAB, patients do not need to be hospitalized, the sectioning time is short, and the complications are relatively minor. It can also save patients with benign lesions from unnecessary operations. The depth of the lesion (microcalcification) in the breast is an important basis for SVAB. This study is exploring that the use of breast computer tomography during the examination process can accurately know the location of microcalcifications (lesions) and determine the depth, so that the doctor can make adjustments during the biopsy to achieve more pathological tissue slices.
