Dental intrusive luxation is a severe dental injury. Managing intrusion injuries for immature permanent teeth is challenging to the clinician because of the poor prognosis and unpredictable treatment outcome. This case report presents the treatment course and long-term follow up of a 7 years 8 months boy who suffered from severe intrusion of left maxillary lateral incisor. Spontaneous eruption followed with active orthodontic reposition was done, and the treatment result was satisfactory after 40 months follow-up.
