Dental trauma often occurs in adolescent, especially at upper incisors. Tooth intrusion or avulsion is much more serious among these dental trauma. In this case, a 13-year-old male fell down from stairs at school, causing upper left central incisor intrusion. After fixation with wire, the patient shook the tooth, so the resin and wire was dislodged. The patient came to our hospital again. We warned the patient not to touch the tooth after FIxation with wire. However, the patient shook the tooth o, he came back hospital the third time on the same day with the repositioned intrusive tooth in his hand. The avulsion tooth was root canal treated extraorally first, then plant the tooth into the socket with fixation. This kind of dental trauma injury's prognosis is very bed in longterm follow. Though the tooth heals well now, it still need to follow for a long time.
