This case report describes a twelve-year old boy with cerebral palsy had his right mandibular second premolar pulp necrotic due to central cusp fracture. The periradicular diagnosis was symptomatic periradicular periodontitis. Instead of intra-canal instrumentation, intra-canal irrigation with sodium hypochlorite, calcium hydroxide medication and filling with MTA were performed. After 6 months, continued development and formation of the root were observed through the periapical radiograph. This case report indicates a more conservative approach may be preferable to traditional Apexification when treating pulp necrotic immature teeth.
