乳臼齒低位咬合之治療：病例報告與文獻回顧

摘要

Infraoccluded primary molar could cause tilting of the adjacent teeth, over-eruption of the antagonists and impaction of the successor premolars. This article presents two severe infraocclusion cases with ankylosed mandibular second primary molars. After extraction of ankylosed mandibular second primary molars and space regained by the lingual holding arch, successor premolars successfully erupted in the 2.5- year and 3.5- year recall, respectively. Treatment modalities of infraoccluded primary molar depends on various clinical conditions. Early recognition, differential diagnosis, the early intervention at the appropriate time are essential for the favorable treatment results.

乳臼齒的低位咬合（infraocclusion）會造成鄰牙傾倒、對咬牙過度萌發及干擾繼生小臼齒萌發，早期適當地介入治療可避免這些併發症。本報告呈現兩個下顎第二乳臼齒低位咬合之案例，經診斷發現這兩顆乳臼齒皆為黏連齒（ankylosed tooth），並已喪失空間維持與咬合功能，且其繼生齒皆有牙胚異位（malposition）情況。綜合分析評估後拔除下顎第二乳臼齒，使用下顎舌側弓線（lower lingual holding arch）進行空間重獲（space regain），分別經過兩年半及三年半療程後，第二小臼齒皆順利萌發，成功地完成咬合誘導。乳臼齒出現齒黏連可能產生諸多併發症，因應各類臨床狀況應採用不同的治療選擇。早期診斷、適當時機的介入治療與合宜的治療選擇，可以達到良好的治療效果。
