目的:瞭解醫院員工代謝症候群之盛行率和發生率及其年度別、年齡別、性別與職務別之差異。方法:採回溯性世代研究法,回溯期間以2013年1月至2016年12月底,期限內曾任職北台灣某醫院參加一次健康檢查以上之員工為研究對象;共得樣本7,175人。結果:代謝症候群2013-2016年累積盛行率為11.2%且與年度別、年齡別、性別與職務別等變項具顯著相關;2015(含)後顯著高於2014與2013年;男性明顯高於女性(17.5%>8.6%);年齡別以>50歲最高達28.3%、其次為40-50歲16.3%;職務別以行政人員之盛行率居高(20.9%),其次為醫師(8.2%)。整體員工(共計641人有兩次以上健檢者)之代謝症候群2013-2016年累積發生率為7.3%,年齡別>50歲發生率達14.1%居首,其次為40~50歲(10.0%),男性高於女性(9.1%>5.9%);多變項分析顯示年齡別、性別與發生率間具顯著相關;,50至66歲間之相對危險比為30歲以下的7.21倍;男性為女性的1.52倍。結論:醫院員工代謝症候群之盛行率和發生率與年齡別及性別有顯著相關,建議醫院針對40歲以上員工提供代謝症候群之預防與治療服務。
Purpose. The study investigated the prevalence and incidence of metabolic syndrome (MS) across the years, age, gender and roles among health care workers. Methods. Materials of this retrospective descriptive cohort study collected through health examination from Jan. 2013 to Dec. 2016 in a teaching hospital of northern Taiwan. 7,175 participants who had at least once health examination (maximum four times) were recruited as the study sample. Results. The overall prevalence of MS was 11.2% in 2013 to 2016 and significantly related to years, age, gender and roles; men displaying a higher prevalence than women (17.5%>8.6%); those who over the age of 50+ had the highest prevalence of MS (28.3%); administrative staff and physicians had higher prevalence of MS than others. The overall incidence of MS among the 641 participants (those who had at least twice health examinations) was 7.3%, displaying a higher incidence than women (9.1%>5.9%). The age of 50~<66+ compare with the age of <30+ had higher risk of MS (RR=7.21, p=.000) and men were at greater risk than women (RR=1.52, p=.046). Conclusion. Both prevalence and incidence of MS are significantly related to age and sex, it is recommended that prevention and treatment programs of MS are highly demanded for those faculties above 40 years old.
