摘要

本專案為運用多元策略提升印尼籍移工鼻胃管管灌技術正確率。若照顧者灌食技術或相關照護不夠確實，易造成吸入性肺炎等合併症，進而影響住院天數，嚴重可能導致呼吸衰竭死亡，對醫療服務品質造成負面影響。經調查印尼籍移工對鼻胃管管灌技術正確率低的原因：語言不通；衛教工具不足；無稽核制度。因此，進行改善策略包括：運用護理治療車上的翻譯軟體、製作印尼版衛教手冊、製作印尼版衛教海報、製作印尼版衛教影片、制訂印尼籍移工鼻胃管管灌稽核制度。本專案推行後結果發現：印尼籍移工對鼻胃管管灌技術正確率由54.17%提升至97.92%；鼻胃管照顧認知率從46.67%提升至97.50%，顯示專案介入措施效果良好，有助於對護理照顧品質之提升。

This report described a nursing experience of 48 years old woman for cardiomyopathy produced by LAMP2 mutations. The nursing period lasted from June 13, 2018 to June 20, 2018, through case manager interviews, observations, listening, and visits, and by using assessment guidelines for physical, psychological, social, and spiritual functioning to frame the nursing-related problems of the patient, including activity intolerance, uncertainty in illness theory, and knowledge enhanced, readiness for cardiomyopathy nursing care. During the treatment period, we educated family and patient about the disease process, complications of disease process, information on medications, need for weighing daily, and when it is appropriate to call doctor. The uncertainty in illness theory, we were given positive encouragement and care at the appropriate time and assist the patient and family members for genetic testing. The process continued to enhance the patient's selfcare skills through continuous tracking over the telephone so that she could proactively undergo treatment and achieve the best possible physical and mental adjustment. We wish to share the nursing experience as a reference for peers when facing similar cases.
