喘症之中西醫治療病例報告

摘要

喘症是以呼吸困難(dyspnea)之症狀爲主，急性或慢性呼吸困難多由於心肺功能不正常引起，而神經肌肉疾病或過敏性疾病也會產生呼吸困難之症狀。中醫對於喘症區分爲實證與虛證，依據證型不同處以中藥。
這是一位87歲的女性病患，平時有高血壓，服西藥已10多年，於2004年8月胸部X光即有心臟擴大的現象，本次因喘促已一周前來就診，並有手抖，手腳無力感，目淚出，疲倦甚，右肩酸痛，咳嗽黃痰量多，並有眠差，口乾，便日一行，便軟，頻尿量多之症狀，診斷爲心肺腎氣虛。治療上以補心肺腎之氣虛爲主，經中西藥治療後病情得到顯著之改善。
其間因患者停中藥後症狀又反復，但服藥後病況獲得改善。由此患者之治療病程可知，應用中醫對於喘症病因病機之認識而處以中藥之治療有不錯之療效。

Dyspnea is defined as difficult breathing. Acute or chronic dyspnea often caused by abnormal heart or lung function. Neuromuscular or allergic diseases also show dyspnea symptom. In Chinese medicine, dyspnea is categorized into excess syndrome or deficiency syndrome, and different treatments are applied accordingly.
A 87 y/o female was diagnosed with hypertension, and has been taking western medicine for about 10 years. Enlarged heart is identified with Chest X ray in April 2006. Her chief complaints are Dyspnea for 1 week with hand tremor, weakness of both legs and hands, frequent tear, fatigue, right shoulder pain, cough with yellow sputum, insomnia, dry mouth, soft stool, frequent urination. After TCM treatment, symptoms and signs were improved. The symptoms were relapsed because patient ceased to take Chinese herbs in the course of treatment. But symptoms were improved again after treatment continued.
In the course of disease and treatment, we learned that effect of Chinese herb in dyspnea is affirmative.
