摘要

本文描述一位熱中暑導致神經功能受損病童，經由出院準備服務提供家屬返家照護需求的經驗，照護期間為2014年7月29日至8月15日，筆者運用生理、心理、社會與靈性之整體性評估結果，確立主要健康問題為：呼吸道清除功能失效、家庭因應能力增進的準備度等問題，聯繫跨領域團隊成員與家屬共同討論，擬定符合病重個別需求之學習計畫，讓家屬可有效、反覆學習相關照護技能，增加家屬照顧的經驗與自信心，並協助申請、提供社會資源等出院準備，使病童得以順利的出院，後續藉由電訪及家訪追蹤、關懷居家照護問題，以延續病童後續的照護及家庭整體之運作。期望藉此病童照護經驗分享，可提供臨床醫療團隊照護之參考。

This article describes nursing experience of providing the family of a young heat stroke patient complicated with neurologic deficits with community resources and household management techniques as needed through discharge-planning services. During the care period, from July 29, to August 15, 2014, we assessed the patient's physiological, psychological, sociological and spiritual status to identify the main problems, including ineffective airway clearance and family resource preparation inability. Based on the patient's assessment and through multidisciplinary teamwork and discussion with the family, we tailored discharge planning to meet the patient's individual needs. The planning not only raised the experience and confidence of the family members in providing care to the patient at home, but also helped them to access eligible social services. After discharge, we called and visited the patient to ask whether there were questions about home care and follow up the situation of the patient. By sharing this experience, we hope that it could offer medical teams with insight into further clinical care.
