碩士班進階精神衛生護理能力導向課程設計之探討

Exploring the Competency-Oriented Master Curriculum Design of Advanced Psychiatric Mental Health Nursing

摘要


因應目前健康照護系統的變革,醫療照護品質要求,近年來進階護理師(Advanced Practice Nurse, APN)的實際性及有效性在其服務的環境中受到重視。護理碩士班亦注重各科領域的進階執業護理師發展,重視護理人員進階實務能力的培養。故本研究整體目的為建構進階精神衛生護理師課程與教學模式,研究者合作進行達到下列分項目標:(一)建立對於進階精神衛生護理能力的期望及評價的機制(二)促進師資的學養(三)建立對學生先備能力評估及因材施教的指導(四)建立優質的課程設計(五)建立改進之回饋機制,本文主要目標即為探討強調能力導向之進階精神衛生護理課程設計。本研究目的在建構進階精神衛生護理課程模式,希望達到下列目標:(一)建立對於進階精神衛生護理能力的指標;(二)課程設計,包含課程及實習目標之明確性。研究方法為行動研究法,研究行動者亦是參與者,由某校五位精神衛生組碩士班教師教育進行行動研究法循環式的教學發展。針對本研究學校97精神衛生護理(一)原有課程及98精神衛生護理(一)進行前趨性研究(pilot study),再以98精神衛生護理(二)課程、99精神衛生護理(一)、98精神衛生護理(三)課程及99精神衛生護理(二)課程為正式的行動研究。執行方法包含:1.共同討論:規劃、經驗分享、教學、設計教學目標;2.課堂觀摩:提昇教學的知能;3.實務指導及案例報告:臨床實務與教學經驗分享,以強化教師實務能力及教學;4.執行評值回饋系統;5.實習場所的規劃與建立教學聯盟關係,並將執行過程做為記錄再以內容分析法分析。研究結果顯示:一、以中華民國精神衛生護理學會向衛生署提出的精神衛生專科護理師規畫書為「進階精神衛生護理師」能力指標,但在學習發展階段中,亦以中華民國精神衛生護理學會建構的「精神衛生護理師實務能力」及「社區精神衛生護理實務能力」為能力指標;二、完成精神衛生護理(一)、(二)、(三)課程教學計畫及單元目標;並由歷史情境觀來討論本文教育行動研究關注的問題,研究成果可提供「進階精神衛生護理」課程設計、能力指標之參考。

In order to satisfy the reform of the current health care system, and fulfill the quality requirements of health care, Recently, the nursing practitioner's practicality has turned to be highly regarded in clinical workplaces. The Master's program in nursing also focuses on the developments in the field of advanced practice nurses, attaching importance to advanced practice ability training of nursing staff. The overall purpose of the study is the construction of advanced course and teaching mode of mental health nurses, and researchers collaborate to achieve the following objectives: 1. establishing advanced mental health nursing capacity expectations and evaluation mechanisms; 2. promoting the learning of teachers; 3. establishing the students' prior capacity assessment and individualized guidance; 4. establishing the quality of curriculum design; 5. establishing improved feedback mechanism; and discussions on the main goal of this article shall be emphasized capacity-oriented advanced mental health nursing curriculum design. This study aims at constructing advanced mental health nursing curriculum model, and tries to achieve the following objectives: 1. establishing an advanced mental health nursing capacity index 2. curriculum design: containing courses and practical objectives with clarity. The study adopts the action research method, in which the research actors are five participants from master's degree programs in mental health class who carry out the cyclic development on action research. With focus on the fiscal year of 2008 mental health nursing pilot study, our study has the courses of 2009(2), 2010(1), 2009(3), and 2010(2) included as formal research action. Executing methods include: 1. discussion: planning, experience sharing, teaching, designing teaching goals; 2. classroom demonstration: improving teaching competence; 3. practical guidance and case studies reports: clinical teaching practice and experience-sharing, to strengthen the substantive capacities of teachers and teaching; 4. perform evaluation feedback system; 5. places of practice planning and establishment of teaching alliances, and recording the conducting process to be analyzed by the content analysis method. Research findings: 1. On behalf of mental health nursing Association of the ROC present to the Department of Health's mental health specialist nurses program books as ”Advanced mental health nursing teacher” index, but during the phase of learning and development, the construction of the ”substantive capacities of mental health nursing division” and ”community mental health nursing practice ability” by mental health nursing Association as an index. 2. Teaching plans and goals; and through the view of historical context to discuss issues of concern to this educational action research, and also the research results can provide ”Advanced mental health nursing” with curriculum design for index reference.
