建構精神衛生家庭護理治療實務能力

Constructing Clinical Competence of Family Nursing Therapy for Psychiatric Mental Health Nurses

摘要


本研究目的為建構精神衛生家庭護理治療實務能力培訓內容及認證的參考,以提升精神衛生護理人員在家庭治療情境中的治療者角色及照護品質。研究採行動研究法進行,由產學界護理專家組成核心小組,邀請臨床資深精神衛生護理人員實際參與研究計畫培訓。研究對象即為臨床實務工作者,包含一位副護理長、三位居家護理師、一位急性病房護理師,資料採內容分析法,藉由種子成員實際培訓情境、會議記錄和核心小組省思的資料,分析實務照顧經驗、家庭互動過程、成員自我省思內容所呈現之操作定義。研究分為準備期、形成期、執行期三階段。研究結果顯示「精神衛生家庭護理治療」之目的為發展以人為本的病患及家屬之家庭護理治療;對象為門診、急診、出院準備、社區精神病患與家屬;情境為門診、急診、出院準備、社區精神病患與家屬的照顧場域;以及實務能力必備要項的操作性定義,包括(一)展現與反思自我成長的經驗(二)具備家庭護理治療之理論(三)執行家庭評估,為評估家庭結構性特質、家庭功能、壓力及調適功能、社會支持系統、家庭健康(四)展現家庭護理治療處置(五)進行家庭護理治療評值。此結果可以增進臨床在家庭護理治療的實務應用,作為實務能力培訓及認證制度之參考,展現精神護理專業價值,並提升精神衛生護理照護品質。

The purpose of the research was to establish the learning content and accreditation reference of the family therapeutic clinical competence of psychiatric health nursing, in order to enhance the therapeutic role and service quality. We have adopted action method in the study. A group of experts were engaged in practice to form a research term, and to invite psychiatric health nurses, including an assistant head nurse, three home care nurses and an acute ward psychiatric nurse. This study used content analysis method through real situations of the seed members receiving training, the recording of meeting, and the reflection data of the expert groups. We analyzed the practical experiences, family interaction process, and members' self-reflection for presenting operation definition. The study went through three stages which were conducted by a period of preparation, a period of formation, and a period of execution separately. The results demonstrated that the aim of the family psychiatric therapy is humanity-nature care for patients and their family. The context involves in the nursing care of outpatient, emergency, discharge planning, community psychiatric patients and their families. The required clinical competence can be concluded with some traits: (1) demonstrating the reflection on the experience of self-growth; (2) equipped with practical experience of family care and treatment theory; (3) able to conduct family assessments, such as the assessments of family structural characteristics, of family functions, of stress and adaptation skills, of the social support system, and of family health; (4) able to provide those families with therapy treatment; (5) to operate therapeutic effects. We believe that this paper's outcome can improve the family therapy by clinical competence with a reference to clinical capacity training and certification system, which displays the value of the psychiatric health nursing profession and enhances the quality of mental health care.
