提升精神科日間病房病人健康體適能之專案

A Project about Improving Patients' Physical Fitness in a Psychiatric Day Care Ward

摘要


背景:本專案35位病人參與前測,其中27位全程參與專案,8位則因出院而退出。27位病人健康體適能之BMI稍重及過重佔66.67%,而柔軟度33.33%、心肺耐力59.26%、肌耐力66.67%落於五分常模之不好分級中。分析原因為:缺乏動機、飲食行為不良、缺乏健康體適能正確認知及病房提供的運動課程不足。目的:提升精神科日間病房病人健康體適能。解決方案:(1)安排分享團體增強動機,(2)運用許願卡及積分表增加動機,(3)安排健康體適能衛教講座,(4)提供健康體適能手冊,(5)安排有氧課程每日30分鐘。結果:BMI稍重及過重比率從66.67%改善至48.15%;不好分級比率改善,分別為柔軟度從33.33%至11.11%,心肺耐力從59.26%至37.04%,肌耐力從66.67%至40.74%,達專案目的。結論:本專案有效改善精神科日間病房病人健康體適能;建議列入日間病房品管項目,並常規安排健康體適能運動及衛教課程,以提升精神病人健康體適能。

Background & Problems: Thirty five patients participated in this project and finished the pre-test, among which twenty seven participants did take part in this project from the beginning to the end, and eight participants withdrew due to the discharge from hospital. Two-thirds of 27 participants (66.67%) in this project were slightly overweight on body mass index (BMI). Besides, 33.33%, 59.26%, and 66.67% of the participants reported having poor physical flexibility, less cardio-respiratory endurance, and bad muscular endurance respectively, which were measured by a 5-point normal scale. The above-mentioned causes were analyzed as follows: the lack of motivation, abnormal eating habits, lack of accurate cognition upon physical fitness, and insufficient exercise sessions provided by the ward. Purpose: The purpose of this project was to improve the patients' physical fitness in the psychiatric day care ward. Resolution: Intervention strategies were designed as follows: (1) arranging groups to share and reinforce patients' motivation; (2) applying wish cards and score sheets to improve patients' motivation; (3) holding educational programs of physical fitness; (4) providing physical fitness pamphlets to patients; (5) scheduling a 30-minute aerobic session per day. Results: The rate of BMI in the slightly overweight range shows improvement from 66.67% to 48.15%; the rate of poor physical fitness in flexibility, cardiorespiratory endurance, and muscular endurance also elevate from 33.33%, 59.26%, 66.67% separately to 11.11%, 37.04%, and 40.74%. The goal of this project is achieved. Conclusions: This project has effectively improved the patients' physical fitness in the psychiatric day care ward. We thus suggest that patients' physical fitness should be monitored regularly and be included in the quality control list of the day care ward. Additionally, physical fitness exercise and relevant educational sessions can be routinely arranged to help psychiatric patients' physical fitness.
