探討情境家庭護理理論治療知能之實例應用

A Clinical Case about Exploring the Contextual Family Nursing With Therapeutic Knowledge and Competency

摘要


本文是於精神科居家護理的訪視過程中,運用情境家庭護理理論於社區精神分裂症病患及其家庭之護理經驗,護理期間自2011年9月6日至2012年5月31日。本文的主要目的在探討「觸」之治療知能,包含互動式溝通、對存在現象的體諒關懷、正向回饋、促進感知之活動及促進問題解決的過程等處置技巧。在每次的居家訪視中,以情境家庭護理理論為依據進行評估,體認到個案家庭在「溝通功能」、「健康照顧功能」與「獨立及依賴功能」的困難,並運用「心」、「識」、「觸」、「智」等治療的元素完成護理過程。結果呈現運用家庭情境護理的處置技巧可促使個案家庭溝通型態由不溝通到平行溝通,甚至投射溝通;個案及其家屬較能互相照顧彼此的健康問題;案大姐可以提供個案情緒支持,讓個案可以工作並面對所遭遇的問題。個案及其家庭經由這樣的介入過程後,能持續以有效的模式溝通,面對家庭的苦難與困難,朝未來邁進。希望藉由此護理經驗的分享,能協助護理人員了解情境家庭護理理論的治療知能及其應用方式,作為未來進行家庭護理治療之參考。

This article described a nursing experience of applying the contextual family nursing theory to a schizophrenic patient and his family in the community, who received psychiatric nursing home's visits from September 6, 2011 to May 31, 2012. According to the contextual family nursing theory, data must be collected during each visit, and then be analyzed and drafted down the difficulties of the patient and his family, such as communication function, health care function, and independent/dependent function. We also used the therapeutic elements of the contextual family nursing theory: mind, acknowledgement, contact, and wisdom in the nursing process. The results include: (1) the communication type of the family changed from no-communication to parallel communication, even projective communication; (2) the patient and his family could take care of each other of their health problems much better; (3) the patient's older sister could give more emotional support to help the patient go to work and cope with difficulties in his job.The purpose of this article was to explore the contextual family nursing with therapeutic knowledge and competency by using interventions, interactive communication, tender care of existential phenomenon, positive concerns, sensory facilitated activity, and accelerating the problem-solving process and so on. We hoped the patient and his family able to maintain the effective communication while facing their pains and distress, and to keep moving forward to the future. We believe that this nursing experience can be shared with other clinical nurses helpful for them to understand the contextual family nursing theory, and be used as a reference of providing family nursing therapy.
