運用自我效能照護一位藥物濫用病患之護理經驗
A Nursing Experience Of Applying Self-Efficacy To A Patient With Substance Abuse
摘要
藥物濫用不僅身體傷害，也會導致自我效能及家庭功能受影響。本文敘述一位女性個案，離婚後藉由施打毒品逃避現實，導致併發肺部疾病住進加護病房之護理經驗。於2013年5月26日至6月10日護理期間，由觀察、會談、及生、心、靈、社會各層面進行護理評估，確立主要健康問題：氣體交換功能障礙、無效性因應能力。運用提升成就表現、諮詢轉介替代經驗、正向言語說服、改善生理與情緒狀態，以增進自我效能為主軸，教導肺部復健改善氣體交換功能；以正向因應技巧面對問題，降低用藥慾望，成功協助個案重返社會。建議毒品防制中心與醫療機構聯繫合作，藉由護理專業即早介入，以醫療團隊整合照護提升照顧成效。

Substance abuse is not only harmful for health, but also incurs an impact upon self-efficacy and functions of a family. This article described a nursing experience of intensive care unit while caring for a female patient who was hospitalized with pneumonia and pulmonary edema due to substance abuse. The nursing period was from May 26th to June 10th, 2013. The physical, psychological, spiritual and sociological data were collected from observation and individual interviews, identifying that the major health problems were impaired air-exchange and ineffective coping strategies. Through enhanced performance accomplishments, vicarious experience referrals, and verbal persuasion, to help ameliorate her physiological and emotional status, the core issue of nursing process would enhance the self-efficacy. We applied pulmonary rehabilitation to improve the patent’s air-exchange function, and facing the problems by using positive response skills, thus, able to reduce the desire of drug use, helped the patient return to the community life. Finally we suggested the application of early nursing intervention and team resource management (TRM), and an increase of the cooperation between the Center of Drug control and Medical institution for better improvement on the care effectiveness.
