運用手機應用程式協助酒癮病人克服心理依賴之照護經驗

A Nursing Care Experience of Using Mobile Apps to Help Alcohol-addicted Patients Overcome Psychological Dependence

摘要


本文描述協助一位因長期失業導致終日藉酒消愁之男性,克服其心理對酒精依賴的護理經驗。護理期間為2017年4月6日至4月27日,運用評估、會談、觀察及閱讀病歷文件等技巧,收集身體、情緒、智能、社會及靈性等五大層面資料,確立健康問題包括:無效性因應能力、家庭因應能力失調及長期性低自尊。整體護理過程中,首先與個案共同擬定戒酒承諾書,再運用手機應用程式來紀錄戒酒歷程,幫助個案減少飲酒,並運用角色扮演方式,協助個案克服酒精渴望之危險情境。筆者以中立不批判的態度,接納個案多次戒酒失敗之經歷,引導其正向思考並找到人生的價值。最後,運用團隊合作維持雙向溝通及家屬參與手機應用程式戒酒等措施,也增進家庭凝聚力並重建個案的家庭支持系統。

This case study describes the nursing experience of assisting an unemployed alcohol-addicted patient with long-term history to overcome psychological dependence on liquor. The nursing care was given from April 4th to April 27th, 2017. Assessment tools such as evaluation, psychiatric interviews, clinical observation, and medical chart review were adopted to retrieve data from five dimensions: physical, emotional, intellectual, social and spiritual dimensions. The nursing diagnoses included ineffective coping, disabled family coping, and chronic low self-esteem. During the nursing care, we first worked together with the client to make a draft of agreeing upon abstention from drinking, and then used the mobile application to record the process of no alcohol to help the client reduce drinking, and as well as using role-playing technique to assist the client in overcoming the tempting situation of craving for alcohol. With a neutral and non-critical attitude, the author accepted the case of repetitive failures in withdrawal of alcohol and guided him to think positively and further find the value of life. Finally, it turned out adopting measures such as teamwork's maintaining a two-way communication and family's participation in cell phone applications for quit drinking is truly helpful; besides, it also increased family cohesion and rebuilt a family support system for individual case.
