照護一位缺血性腦中風病患的護理經驗

Experience of Caring a Patient with Ischemic Stroke

摘要


本文乃探討一位因腦中風後,導致肢體活動功能障礙之息者的調適過程及行為反應。照護期間(96/2/11-96/2/25)筆者運用Gordon十一項功能性健康型態評估,經由會談、觀察、身體評估等技巧,收集主、客觀實料,確立個案主要護理問題有身體活動功能障礙、焦慮、潛在危險性跌倒和便秘等。藉由與個案及家屬的會談建立良好的互動,給予正向的支持,並採持續性的關懷、傾聽及鼓勵個案說出內心的不安,使得情緒得以紓解,並激發個案自我學習的動機,克服疾病導致的後遺症,進而提升自我照顱能力及身體功能的恢復。而筆者照護此個案間深切的體會護理人竟在照護病人時,適時給予適切性且個別性的護理指導,發揮護理的獨特功能,方能解決個案的問題,以協助個案度過生病的低潮期,進而適應疾病,學習自我照顧以提升生活品質。

The purpose of this article is to explore the adaptation and behavioral responses of a patient with ischemic stroke who encountered impaired physical mobility. The nursing period was during from Feb 11, 2007 to Feb 25, 2007 and the nurse performed conversations, observations and physical assessments in order to collect data. By applying the eleven functional health patterns as described by Marjory Gordon on this case, it was found that the nursing assessment clarified certain nursing issues including impaired physical mobility, anxiety, potential risk for falling-down and constipation. For these reasons, the author provided positive support by continuous caring, listening and encouraging express to relieve the notional stress among the patient's and family's interaction. The author worked up the patient's learning motivation, conquering the sequela, promoting self care ability and restoring body function. The nurses should give individual intervention to patient, promote unique function, and solve the patient's problems. We should help patient pass through low tide in disease, adapt disease, enhance self-care abilities and improve quality of life.
