照顧一位缺血性腦中風病人之護理經驗
Nursing Experience on the Care of Patient Diagnosed with Ischemic Stroke
摘要
本篇係探討一位69歲男性，因缺血性腦中風導致其身體功能降低及焦慮之護理經驗。筆者自2012年5月23日至2012年5月30日止，藉由會談溝通、觀察及調閱病歷、配合臨床理學等收集資料。運用Gordon十一項健康功能進行評估，確認個案護理問題為：身體活動功能障礙、自我照顧能力缺失及焦慮。筆者運用治療性溝通技巧，與個案建立信任感及良好的護病關係，給予個案正向支持，藉由醫療團隊整體性的照護，協助個案與家屬共同面對問題，訂定護理計畫提供適當之照護。例如：暸解疾病成因、疾病的治療及復健的重要性，提供適當自我照護，進而降低其焦慮感受。期盼此護理報告，增加臨床醫護人員對於缺血性腦中風患者認識與了解，運用具體有效護理措施解決個案之護理問題，增加其自我照護能力，降低再住院的機率，進而提升病人整體之生活品質。

This article describes the one-week nursing care experience of a 69-year-old male patient with reduced physical function and anxiety due to ischemic stroke. Patient data was collected through interviews, active listening, physical assessment, and medical record review. Gordon's 11-item Functional Health Pattern identified the care problems, which included dysfunction of physical activity, self-care deficiency and anxiety. Therapeutic communication established a sense of trust and good nurse-patient relationship that provided support and a multidisciplinary team approach to the patient and his family. Interventions included patient teaching of disease and treatment process, the importance of rehabilitation, and assistance with self-care activities that contributed to the reduction of anxiety. The improvement of nursing knowledge on the care of patients with ischemic stroke may increase effectiveness of care, improve patients' self-care ability, reduce rate of readmission and improve patients' quality of life.
