居家失能老人及其家庭運用社會資源之照護經驗

The Nursing Experience of Caring a Disable Elder and Her Family and Using Social Resource: A Case Report

摘要

「在地老化」促使失能老人回歸熟悉的家庭接受照護，但對家庭及其成員引發的衝擊卻不得等閒視之。本篇在論述中風失能老人及其家庭運用社會資源之照護經驗，護理期間自97年6月20日至97年9月30日達三個多月之久，筆者藉由家訪、電訪、觀察、會談等方式，透過傾聽、關懷、同理心等技巧與個案及家屬建立信任護病關係。護理過程中發現個案及其家庭出現「自我照顧能力缺失、高危險性肺吸入、無望感、照顧者角色緊張」等健康問題。筆者運用護理專業知能滿足失能引發的照護需求，提昇主要照顧者居家照護技能的品質；也以家庭為中心的理念，協助家庭成員協調照顧事宜的分工；並運用多元社會資源，增進個案居家照護品質。筆者希冀本篇個案報告之照護經驗，能提醒居家護理人員以「家庭」為照顧對象，整合照護資源，減緩照顧者負荷，使失能老人晚年的照護契合「在地老化」的本土化社區照護的特色。

Caregiver loading is an important issue in th longterm care system, which maybe induce psychosomatic impact. The purpose of this case report is to describe the nursing experience and effectiveness of implementing case management for a caregiver who caring three disable families. The nursing duration was from 20th June 2008 to 30th September 2008 through home visit and phone conversation, we used observation, interviewing, physical examination, and comprehensive nursing assessment to identify the health problems. Major health problems were self-care deficit, risk of aspiration, hopless, caregiver role strain. The authors applied professional nursing skill and used multiple social resources to help disable elder and caregiver to face the impacts of careing loading. Finally, the disable elder self-care ability improved and caregiver loading descreased through family meeting and using social support. We hope this experience of case management may provide some helpful information for us to share in caring for such cases.
