偽裝成甲狀舌管囊腫之異位甲狀腺－案例報告
Ectopic Thyroid Gland Masquerading as Thyroglossal Duct Cyst-Case Report
摘要
異位甲狀腺其實並不多見，大多位於舌根或舌下，僅少數會位於頸部前方中線。曾有文獻報告有人把異位甲狀腺當成是甲狀舌管囊腫直接手術切除，當術後發生甲狀腺功能低下時，才發現是該患者體內唯一的甲狀腺，故術前仔細的評估實為重要。一40歲女性，因意外發現頸部前方中線處有一顆腫塊而求診，初步臆斷為甲狀舌管囊腫，原本計畫Sistrunk氏手術。但是，電腦斷層證實為異位甲狀腺，由於患者並無相關的臨床症狀，也不影響外觀，建議保守追蹤即可。

Ectopic thyroid glands are uncommon. Most of them locate at tongue base or sublingual, and only rare curiosities locate at midline anterior neck. In the literature, someone has misdiagnosed an ectopic thyroid gland as a thyroglossal duct cyst, and excised it. After surgery, the patient suffered hypothyroidism, and only thyroid gland was confirmed; therefore, a detailed pre-operative evaluation of midline anterior neck mass is mandatory. A 40-year-old woman presented with a midline anterior neck mass. Thyroglossal duct cyst was impressed and Sistrunk's operation would be scheduled. However, computed tomography demonstrated the mass was ectopic thyroid gland. Because she did not have any associated symptoms and the mass did not influence the look, conservative treatment and regular follow up were recommended.
