健保總額預算期間醫檢師的工作感受及工作壓力

摘要


背景及目的:全民健保實施總額預算後,人力精簡,醫療人員工作量、壓力增加。探討護理人員、醫師及治療師工作壓力的文獻非常多,針對醫檢師工作壓力的比較少,本研究針對醫事檢驗師(以下簡稱「醫檢師」)的工作成受及工作壓力進行調查。方法:本研究採橫斷式問卷調查,對桃園縣醫檢師以分層比例隨機取樣,邀請醫檢師填寫問卷。量表採已具信效度的健保總額預算前後工作感受改變(WPGB)及醫療工作的壓力源感受量表(QMWS)。結果:於2006年7-12月得有效問卷76份(51%)。WPGB總分為29.45(百分位65.4),單項前三項為工作時間、文書處理量、工作量,影響因素只有月薪。QMWS總分為32.47(百分位67.6),單項前兩項為應付醫院評鑑工作、面對醫療糾紛,影響因素則有WPGB及醫院層級。結論:全民健保總額預算期間,醫檢師的工作感受及工作壓力大。月薪為工作感受的重要影響因素,工作感受及醫院層級則是工作壓力的重要影響因素。

Background and aims: After the global budget payment system of the National Health Insurance was implemented, hospital staff were cut and workloads for existing staff were increased. There are many studies which report on job stress among nurses, physicians, and therapists, but the current literature lacks studies on stress among medical technologists (MTs). This study aimed to survey work perception and occupational stress among MTs, and the relationship between them. Methods: A cross-sectional study with a stratified and proportional random sampling in Taoyuan was used and MTs were invited to fill out a questionnaire on work perception of global budget (WPGB) as well as a questionnaire on medical worker's stress (QMWS). Results: From July to Dec, 2006, 76 valid questionnaires (51.0%) were obtained. The total score of WPGB was 29.45 with a percentile of 65.4. The first three high-ranked items for WPGB were: working hours, paper work, and workload. The significant factor of WPGB was salary amount. The total score of QMWS was 32.47 with a percentile of 67.6. The first two high-ranked items for QMWS were: preparing the hospital for accreditation, managing medical disputes or lawsuits. Significant factors associated with QMWS were WPGB and hospital level. Conclusion: During implementing global budget payment system, MTs' work perception and job stress were very high. Salary amount affected MTs' work perception. Hospital level and work perception affected MTs' job stress.
