本專案目的在降低早產兒醫護處置後低體溫率及縮短體溫回升時間。專案實施前早產兒於醫護處置後低體溫率為80.2%,且須47分鐘回升至正常體溫。分析原因為:缺乏早產兒體溫照護的教育訓練、體溫照護標準不合宜、未執行體溫照護的品質監測、保暖相關設備缺乏或效果差。針對原因提出措施:加強護理人員體溫照護的教育及宣導、修訂早產兒體溫照護標準、定期監測體溫照護技術、改善及創新保暖設備。專案實施後,早產兒於醫護處置後之低體溫率降至16.1%,體溫回升至正常時間為14分鐘,同時護理人員體溫照護執行率、認知正確率皆提高,達到改善醫護處置後低體溫之情形,顯現改善措施有效,不僅增加護理人員的知能,更使早產兒獲得良好的照護品質。
Performing routine procedures on premature infants in the Neonatal Intensive Care Unit frequently leads to transient hypothermia. We undertook a project to decrease the incidence of hypothermia and to shorten the time needed to return to normal body temperature if hypothermia occurred. Before the project, the incidence of hypothermia in premature infants after routine procedures was 80.2%, and the average delay until return to normal body temperature was 47 minutes. The causes of the problem were determined to be lack of a specific training program and nursing standards, inadequate monitoring of the babies' temperature, and ineffective warming equipment. The project was designed to specifically address each of these deficits. After its implementation, the incidence of hypothermia among premature neonates decreased markedly to 16.1%, and the time to return to normal body temperature was reduced to 14 minutes. The project enhanced the nurses' knowledge and efficient use of measures to improve quality of care for these infants.
