Improving Psychiatric Outpatient Nursing Instruction

摘要


本專案旨在提高精神科門診護理指導執行率,統計單位2008年1至6月護理指導執行率平均為16.3%,分析問題為護理人員專業知識不足,擔心影響醫師看診,指導單張不適用及評值困難等。針對問題提出解決方法為舉辦在職教育、修改指導單張、制定護理指導作業標準,並製作醫師護理指導轉介單及告示牌等,實施後執行率提升至68.3%,且經自製的評值單評值,病人及家屬在指導後全數答對率為86%;並對同仁進行調查,護理人員88.7%認為評值單對評值有幫助,83.5%認為轉介單對執行指導有幫助,對護理指導作業標準滿意度為88.7%;醫師97.1%認為護理指導對看診時有幫助,且願意繼續支持轉介病人。經專案改善後更凝聚醫護團隊共識,共同提升照護品質。

We designed a project to improve attendance at nursing instruction programs for patients in the psychiatric outpatient department. A survey from January to June at 2008 indicated that only 16.3% of patients attended nursing instruction. Major problems hindering participation included inadequate professional psychiatric knowledge among the nurses, concerns about interfering with treatment by psychiatrists, a complex and confusing educational handout, and difficulty in evaluating the process. Strategies to improve the situation involved initiating in-service continuing psychiatric education for the nurses, revising educational handouts, modifying instruction sheets, establishing standard operating procedures for nursing instruction programs, designing physician referral sheets, and posting clear information about the programs. After implementation of these measures, the rate of attendance improved to 68.3%. Patients and their family members achieved an average of 86% on a self-assessment tool after attending the instruction. When the nurses were surveyed, 88.7% thought the self-assessment tool was helpful for evaluating the patients' and family members' knowledge, 83.5% thought the physician referral sheets were helpful in improving attendance, and 88.7% were satisfied with the procedures. Among physicians, 97.1% thought the nursing instruction was helpful for the patients and intended to continue referring them. This project enhance the treatment given by physicians and nurses, improving the quality of psychiatric outpatient care.
